You have received a recent diagnosis, or, already have either Type 2 Diabetes or Prediabetes, meaning that you are experiencing insulin resistance and thus have high blood sugar – if untreated, this will lead to health complications, such as a stroke or heart attack, further on down the line. As a result, it is particularly important that we manage the high blood sugar, whilst at the same time, work on reversing the Diabetes.
The first step in this will be for you to understand how your diabetes has happened, and more importantly, how to reverse the condition, as well as the treatment options available to you.
To to do this, please take your time and read through the various pages and resources on the website, once you have done so, kindly fill out the “My Treatment Form”. Upon submission of the form, it will be sent to a secure NHS email, where we will make note of your choice and contact you to book in an appointment so that we can help you treat your diabetes.

The form below is all about what choice of treatment you would like to proceed with, after reading all the information available on our website - this is a choice that is made by you, for you.
The form requires information such as your name, date of birth, telephone number, the resources/webpages you have accessed, and then your preferred treatment method (out of those listed on the "Treatment Options" dropdown menu).
For example, if you would like to go on a Very-Low Carbohydrate - Cambridge Diet , then please type "Very-Low Carbohydrate - Cambridge Diet or VLCD - Cambridge Diet" in the 'My Preferred Treatment' section.

If you select Bariatric Surgery as your treatment choice and are not eligible, we will ask you to make another choice regarding your Treatment.

After submitting the form, you will be contacted by a member of staff from the surgery who will discuss your next steps in more detail and book in an appointment for you with our Health and Wellbeing Coach - starting and helping you on your journey to reversing your diabetes!
